Researchers often face challenges managing large volumes of articles while ensuring accuracy and adherence to methodological rigor. Accelerating this process without compromising quality is essential for advancing evidence-based practices.
Methods:

  • Tool Evaluation: Assessed SWIFT-ActiveScreener, a machine learning-driven tool designed to expedite SR screening using predictive algorithms and active learning.

  • Study Context: Conducted within the context of a large SR focusing on mental health treatment outcomes for PTSD.

  • Participants: 18 screeners (a mix of novice and experienced individuals) reviewed over 10,000 articles during the title and abstract screening phase.

Problem: Military mental health care is an important area of focus, with ongoing efforts aimed at effectively supporting Canadian Armed Forces (CAF) personnel. Despite organizational efforts to encourage help-seeking, a significant challenge remains in retaining individuals in mental health treatment, as premature discontinuation can undermine clinical outcomes.
Methods: This study analyzed data from the 2013 Canadian Forces Mental Health Survey, a cross-sectional, population-level survey of CAF personnel. The analysis explored the prevalence of mental health treatment discontinuation among Regular Force and Reserve Force members, examined reasons for discontinuation, and applied logistic regressions to identify key factors associated with treatment non-completion.
Key Findings:

  • Among Regular Force members, ~20% sought mental health treatment in the past year. Of this sub-group, close to 40% discontinued all forms of treatment within the same year. Notably, only ~25% of those who discontinued reported doing so because they completed the recommended course of treatment. Similar patterns were found among ResF personnel.

  • Common reasons for non-completion included feeling better, thinking treatment was ineffective, or discomfort with the professional approach.

  • Factors like higher education, marital status, childhood maltreatment, and lower social support increased the likelihood of treatment non-completion.

Impact and Implications: This study highlights the complexities of military mental health services provision and offers the first nationally representative analysis of treatment discontinuation in a Canadian military population. These findings highlight the need for tailored mental health interventions in military settings that address individual and systemic barriers to treatment retention. Building trust in therapeutic approaches and fostering strong provider-patient relationships are critical.
Next Steps: Future research should explore the impact of therapeutic engagement, assess long-term outcomes of discontinued treatments, and develop strategies to improve mental health treatment adherence in military populations.

Problem: Military personnel often face potentially morally injurious events (PMIEs) that may lead to moral injury and/or psychiatric disorders, such as PTSD or depression. It is unclear whether concerns over confidentiality, career repercussions, and potential legal risks may deter them from seeking help or disclosing these experiences.
Methods: Using a vignette-based experimental design, 335 Canadian Armed Forces Veterans were presented with hypothetical scenarios involving PMIEs or traumatic events which can lead to PTSD. The vignettes varied by trauma type, confidentiality level, and military status (active vs. released) to assess participants' likelihood to seek help, disclose trauma, and perceive career and legal risks.
Key Findings: The study found that participants were less likely to seek mental health support and disclose trauma details for PMIEs than for conventional PTSD scenarios, especially when confidentiality was ambiguous and participants were imagined as still in service. Higher perceived career and legal risks strongly correlated with a decreased willingness to disclose.
Impact: This study underscores the importance of confidentiality assurances and the need for policies that explicitly protect service members' privacy in mental health contexts, especially in morally complex cases. These findings can guide improvements in military mental health services, making treatment more accessible and trusted.
Implications: Enhanced confidentiality policies and education around the handling of PMIE disclosures may encourage military personnel to seek mental health care more openly. Addressing these barriers is essential for promoting help-seeking behaviours, particularly among those at risk of moral injury.

Problem: Treatment-resistant depression (TRD) remains a challenging condition, especially in military populations with comorbid PTSD and chronic pain, where traditional treatments often prove insufficient.
Methods: This case report explores the use of transdermal buprenorphine in a 36-year-old military Veteran with PTSD, TRD, and chronic pain. The patient’s treatment included multiple antidepressants and therapies, with buprenorphine introduced after limited response to previous interventions.
Key Findings: Buprenorphine treatment led to significant improvements in depressive symptoms, including increased energy and reduced anhedonia. Additionally, the patient was able to discontinue hydromorphone, previously prescribed for chronic pain, with no withdrawal symptoms.
Impact: The case adds to evidence that buprenorphine may have dual benefits in managing both mental health and chronic pain, highlighting its potential for use in military and Veteran populations facing complex health challenges.
Implications: These findings suggest that buprenorphine could be a valuable addition to treatments for TRD in military patients, especially when chronic pain and PTSD are present, encouraging further research on its broader clinical applications.
